Plot Summary

Eleven years after the events in Burma, John Rambo lives a quiet life on his late father’s horse ranch in Arizona with his friend Maria Beltran and her granddaughter Gabriela. When Gabriela learns the whereabouts of her estranged father in Mexico, she secretly travels there, only to be betrayed by her friend Gizelle and kidnapped by the Martinez brothers’ cartel. The cartel forces Gabriela into sexual slavery and drugs her repeatedly.

Rambo, upon learning of Gabriela’s disappearance, heads to Mexico to find her. He is ambushed and brutally beaten by the cartel, who mark his face and vow to further harm Gabriela. Rambo is rescued by Carmen Delgado, an independent journalist investigating the cartel after her own sister’s murder. After recovering, Rambo raids a brothel, kills several cartel members, and rescues a heavily drugged Gabriela. Tragically, she dies from an overdose on the way home.

Consumed by grief and rage, Rambo sends Maria away and prepares his ranch for an all-out assault, rigging it with deadly traps. He returns to Mexico to kill Victor Martinez, decapitating him to lure the cartel’s remaining leader, Hugo, and his men to Arizona. In a violent showdown, Rambo’s traps decimate the cartel. Rambo saves Hugo for last, killing him by cutting open his chest and ripping out his heart.

The film ends with a wounded Rambo sitting on his porch, vowing to keep fighting and preserve the memories of his loved ones. In the credits, flashbacks from previous Rambo films are shown as he rides off into the sunset.
